紐約州NCLEX RN考試申請 (New York RN Application Steps for non-U.S. citizen- take Taiwan Nurse application for example)

Hey there I am from Taiwan, my education background is 3 years in senior high school and then 4 years in Nursing major. The way to fulfill those forms may vary depends on the individual. Hope this article will help you to finish your application.

First thing you should know is this NYSED website http://www.op.nysed.gov/prof/nurse/nurseforms.htm
這是NYS nursing的網站: http://www.op.nysed.gov/prof/nurse/nurseforms.htm，你必須得跟他打交道。

Steps 1: child abuse certificate and infection control certificate. 
There are many courses you can chose on the website. There is no official paper certificate, just download and print them yourself and sent them to the NY board. I heard from others that it is not necessary to send the certificates. Yet I wrote the letter to ask, and the answer is "Download copies of the certificates and mail them to us with the Form One and fee".
第一步:取得孩童暴力及感控證書，是不會有實體證書的，所以千萬別等待證書寄到妳家，當初我有寫信問child abuse課程的開課者關於證書該如何寄給紐約護理局知道，得到的回覆是她們會主動幫你email到紐約護理局，不過為了謹慎，我又寫了一封信到紐約護理局去詢問，他們的回覆卻是要把這兩張證書隨form1(步驟二的form1)及費用143美金(我是用匯票如上圖)一起寄到紐約護理局，所以還是寄吧！

2014 /11 取得child abuse certificate(我選的這個課是免費的，有課後測驗，但很簡單)
取得infection control certificate(這個30美金，題目加填寫滿意度問卷)

Step 2: download and notarize the Form 1.
Download the four-page-form 1. The pictures below show the things you should notice. For example, the 26th column should left blank till the notary check the whole documents. The receipt below is where I notarize my document. Here is the link: http://www.notary-public.com.tw/index2.htm.
第二步: 下載FORM 1下來寫吧，共四頁，填完後去公證，我當初只填寫了一份，然後去公證所，感覺很多人去那邊弄這份文件，非常有經驗，她直接就幫我印了三份，注意字一定要夠黑，複印起來才清楚，她會還給你兩份，你就把一份寄給紐約護理局、一份自己留存。記得第四頁的最後一格26一定要保持空白喔！到公證所才寫，她印好三分後，你就在三份的26格簽名、壓日期。

Send the Form 1, money order and your certificates to NY BOARD.
接著就把以上的文件都寄出給紐約護理局吧！因為有匯票，所以我有保值，但我非常後悔花了台幣490元寄了速件，因為我快兩年才收到ATT，才可以預約考試。


Step 3: Register an account on this CGFNS website-http://www.cgfns.org/services/credential-verification-service-for-ny-state/
This is the institute to  verify the reality of your document such as the diploma. 

Step 4: finish and get the CVS form notarized
There are many service. Click on the CVS(CredentialVerification Service for NYS). After fulfilling the form online, you should print and take them to notarize as well. I personally pay with credit card, you can choose whatever you want. 
加油！然後就是最後一步了！到CGFNS(就是一個幫你審查妳的文件是不是真的的一個機構)(http://www.cgfns.org/services/credential-verification-service-for-ny-state/)，註冊一組妳的帳號，然後
她有好多種不同服務，就選 :CVS(CredentialVerification Service for NYS)，填完cvs APPLICATION後一樣要公證，然後付清美金390元，我當初是線上刷卡。
You should finish the document below, yet these are not required to notarize. 
最後還有兩張文件(Authorization for Verification of Academic Records/Transcripts 、Authorization for Validation of Licensure/Registration) ，不用公證，但要一起寄給cgfns
 (半年後才收到CGFNS說已經驗證完畢！)
  終於寫完了，天都亮了！！！疲勞！
